Finasteride (brand name Propecia or Proscar) and minoxidil (brand name Rogaine or Regaine) are the only two treatments for hair loss that are approved by the medical regulators in the UK, USA and other countries- i.e. they are the only two treatments that are proven to actually work!
Finasteride is an oral drug that stops the male hormone, testosterone, being converted into a more potent form called DHT. DHT isn't 'essential' for any biological functions after adolescence, but it binds with receptors in the the hair follicle causing scalp hairs to progressively shrink and die. DHT also plays a role in prostate problems in later life.

It was originally an oral drug used for the treatment of high blood pressure, but it was accidentally discovered to promote hair growth. Subsequently the manufacturer produced a safer topical form and had it approved for the treatment of alopecia. No-one knows exactly how it works, but it is suspected to increase the supply of nutrients to the hair follicle, causing better growth.
